1.สำเนาหนังสือรับรองผลการเรียนในระดับปริญญาตรี (Transcript) 1 ฉบับ และ สำเนาปริญญาทันตแพทยศาสตรบัณฑิต หรือเทียบเท่า หรือหนังสือรับรองคุณวุฒิ พร้อมลงนาม รับรองสำเนาถูกต้อง
Certified copy of degree certificate. A certified copy of academic transcript stating the graduation with Doctor of Dental Surgery degree from the University accredited by the Ministry of Education and/or the Dental Council of Thailand, and any higher degree(s) in dentistry. (Certified copy with applicant’s signature is acceptable)

For international Students
A letter of convert grade average to GPAX 4.00 system (certified true copy in the document by the applicant with specify text “I acknowledge and agree to the above guideline. Hereby, I certify that the information in this report is entirely true.” )

2.สำเนาผลสอบภาษาอังกฤษ ตามที่ได้ระบุไว้ใน คุณสมบัติของผู้มีสิทธิ์สมัคร พร้อมลงนาม รับรองสำเนาถูกต้อง
Result of English Proficiency achievement in one of the official English language proficiency test scores that meet the following criteria:
2.1 CU-TEP 55 or above
2.2 TOEFL (Institutional Test Program: ITP) 498 or above
2.3 TOEFL (Internet-based Test: iBT) 60 or above
2.4 IELTS 4.7 or above
taken within 2 years. The examination scores must not exceed more than 2 years since the report date until the announcement of exam results. The test scores must be valid for at least 2 months in order to be approved by the Faculty Executive Committee.
(Applicants with CU-TEP score of at least 30 or TOEFL ITP score of at least 400 or TOEFL iBT score of at least 32 or IELTS score of at least 3 may be admitted to the program but they have to meet the language proficiency requirement of the graduate school before graduation.) Applicants who obtain the CU-TEP score less than 30, TOEFL ITP less than 400, TOEFL iBT less than 32 and IELTS less than 3.0 are not considered for admission.
3.สำเนาบัตรประชาชนหรือสำเนาบัตรข้าราชการ 1 ฉบับ พร้อมลงนาม รับรองสำเนาถูกต้อง
Copy of identity card or government identification card (Certified copy with applicant’s signature is acceptable)
4.สำเนาใบอนุญาตเป็นผู้ประกอบวิชาชีพทันตกรรมในประเทศไทย และเคยประกอบอาชีพในสาขาวิชาทันตแพทยศาสตร์
มาแล้วไม่น้อยกว่า 1 ปี นับถึงวันที่ 1 สิงหาคม 2569 พร้อมลงนาม รับรองสำเนาถูกต้อง
Certified copy of dental practice permit. (Certified copy with applicant’s signature is acceptable).
5.หนังสือรับรองประสบการณ์การทำงาน
At least one reference letter of working experience in dental practices.
6.หนังสืออนุมัติให้ลาศึกษาต่อ จากต้นสังกัด (เฉพาะผู้สมัครที่มีต้นสังกัด)
Letter of leave permit for further education from the applicant’s original affiliation (for those with affiliation)
